Dear Client: 

In the past, our policy was to match clients based solely on physical descriptions; however we have now absorbed another match-making company whose policy was to show photos. We have decided to allow clients to view photos, on request, on a trial basis. Each client must sign an agreement (see below) to either allow Misty River Introductions to send photos to clients and/or request to have photos sent for viewing.

AGREEMENT FOR PHOTOGRAPH VIEWING

Agreement to allow Misty River Introductions to mail or e-mail photographs to interested parties upon request.

 I, ______________________________, hereby agree to have Misty River Introductions Inc. send my photo to a client, on request as follows: Please indicate one choice

(1)   on a case-by-case basis (   ), OR

(2)   at our discretion (   ).

I, ______________________________, hereby agree to pay  $10.00 + GST (ten dollars plus GST) to Misty River Introductions Inc. for each photo of a client that I request be sent to me via mail or e-mail.  I authorize Misty River Introductions Inc. to debit my credit card upon instruction for each photograph requested.
